When you need employees to formally acknowledge a company policy, safety standard, handbook update, or regulatory compliance document, you can easily build digital attestations using custom Surveys or Quizzes in BizLMS.
This guide walks you through setting up an attestation form, choosing the right tool for your sign-off type, and tracking learner acknowledgments.
Step 1: Choose the Right Tool (Survey vs. Quiz)
BizLMS offers two formats for gathering learner input: Surveys and Quizzes. Choosing the right format depends on how you want to collect your sign-off:

Whether attached to custom content (like a PDF handbook) or delivered as a standalone requirement, follow these setup recommendations:
Option A: Open Text Signature (Survey Format)
Select the Survey Option.

Add a Single Choice / Checkbox question for the policy statement:
Question: "I acknowledge that I have read, understood, and agree to abide by the Employee Handbook."
Options:

Add a Short Answer / Open Text question for the signature:
Question: "Please type your full legal name below to serve as your electronic signature."
Save and attach the survey to your custom content module or learning path.
Option B: Checkbox Confirmation (Quiz Format)
Select the Quiz Option.
Set the Passing Score to

Add a Single Choice question:
Question: "By checking the box below, I confirm that I have reviewed and agree to the Safety Policy."
Answer Choice:
I Agree and Acknowledge(Mark this as the Correct Answer).
Save and publish.
Step 3: Tracking & Exporting Attestation Records
Once learners complete their sign-off:
Navigate to your Custom Library
Select your attestation resource to view completion rates.
Click Export to Excel / CSV to pull a full audit log containing learner names, timestamps, and exact responses (including typed signatures) for compliance reporting.
Admin Pro-Tip: For formal policy rollouts, combine a uploaded PDF module (e.g., your updated Employee Handbook) with an attached Survey/Quiz attestation in a single Learning Path. Set the attestation as the final required step to ensure learners review the material before signing!
